It's a tool with an uneven end (usually rubber) and you push down on the fruit (including the peel) and squeeze the juice out of it. :)

I had to check out this recipe...my favorite martini...well two...is a lemon drop in the summer and a choco-tini in the winter! Mmmm! looking good! ;)
